What does an example like a colleague spending two years at the Misch Institute and never doing an implant case actually reveal?

That clinical capability, no matter how deep, sits unused without the marketing and selling infrastructure to surface and convert the cases the capability was built for. The example is instructive and not unusual. A dentist invested two years of weekend coursework, substantial tuition, and serious intellectual effort to acquire implant placement and restoration expertise through one of the most respected training programs in the field. The clinical skill at the end was real. Back at the practice, no implant cases ever materialized. The marketing infrastructure that would have surfaced implant candidates from the local population did not exist. The selling system that would have converted those candidates into accepted treatment did not exist. The clinical training, however elite, had no operational layer underneath it to translate into actual cases. The doctor concluded the training had been an interesting professional education experience, which is generous to the situation. The fairer framing is that two years of training represented a substantial sunk investment that produced no commercial return because the surrounding business infrastructure was never built. The pattern repeats across the profession.
