How do I make sure my team actually uses the phone slip?
Make it impossible not to. The single most common excuse I hear when I ask why the slip was not used is, I did not have it with me. So remove that excuse entirely. Copy the slip onto colored paper, pick one color and keep it, pink if you like, just stay consistent. Print a stack. Then leave them out and visible next to every phone where someone could walk by and schedule an appointment. This is where doctors fight me, because they hate paper sitting out on the desks. Get over it. Do not put the slip in a folder, a binder, a shelf, or a drawer. It must be visible and grabbable at every station. Accountability often comes down to design. If the right behavior is the easy behavior, your team will actually do it.
