It costs you real money, and you should know the exact number. Free to the patient is never free to you. Sit down and figure out your overhead per hour of doctor time, then post that number on the whiteboard where the whole team watches the monitors. When everyone sees what an idle consult hour costs, they treat that time differently. They fill it. They prep for it. The point is not to scare you out of giving time away. It is to give it away on purpose, knowing the price, so you use it sharply. A consult slot you waste is gone forever. You can always make more money in this life. You cannot make more time.
