What does the highest complex dental case fee territory actually look like?

Treatment costs spiral up to and beyond $100,000 to $150,000 for the most extreme cases, especially those requiring iliac crest grafts to rebuild bone height before implants can be placed. The territory above $100,000 is reserved for cases at the edge of what the profession can deliver. Both-arch implant-supported restorations with full ceramo-metal work, complex occlusal reconstruction at proper vertical dimension, extensive grafting to address bone that has resorbed past the point where standard implant placement is possible, and the multi-modal coordination of surgical, restorative, and prosthodontic phases. The fees reflect both the clinical complexity and the surgical capability required to deliver them. Iliac crest graft work in particular requires surgeons whose training and experience put them in a very small global cohort. Most patients in this fee territory cannot or will not pay it out of pocket, even when their dental situation genuinely warrants it. The cases that do go forward typically involve patients with substantial wealth, severe enough symptoms to justify the spend, and the time to commit to a multi-year treatment process.
